This is what we do:

We bring healthcare to your loved ones home. In the hospital or nursing home, you are surrounded by 24/7 support. When you leave the hospital and return home, the support you received at the hospital or nursing home is not available at home. This is where we come in. That is, we help you make the transition from hospital or nursing home to your home by bringing the nurses, therapists (physical, occupational, and/or speech), or medical social workers to your home. Our goal is to empower your loved ones to become more independent, active, and engaged in their healthcare. We help coach your loved ones to manage their disease, and recover after surgery.

Your Care Team

Skilled nursing services are given by either a registered nurse (RN) or a licensed practical nurse (LPN). If you get services from a LPN, your care will be supervised by a RN. Home health nurses provide direct care and teach you and your caregivers about your care. They also manage, observe, and evaluate your care. Examples of skilled nursing care include: giving IV drugs, shots, or tube feedings; changing dressings; and teaching about prescription drugs or diabetes care. Any service that could be done safely by a non-medical person (or by yourself) without the supervision of a nurse, isn’t skilled nursing care. Your first visit will be a comprehensive evaluation of your overall health.

Skilled therapy is given by physical therapists, occupational therapists, or speech therapists. Their goal is to improve your mobility and feel stronger so your loved one can enjoy life, become active and independent. The therapist will coordinate with your doctor to develop, implement, manage, and evaluate your customized care plan. 

The medical social worker will assess the social and emotional factors associated with your care. Some of the services the medical social workers will perform are:

  • Social assistance
  • Meals on wheels
  • House-keeping
  • Community resource planning and coordination
  • Assistance with applications for financial aid
  • Advance directives

Our clinical pharmacist will work with your doctor and develop a customized drug schedule that:

Your nurse and pharmacist will talk to you about:

  • How to get the most benefit from your medications
  • How to best take your medications
  • Answer questions about your medications, its' cost, and possible side effects
  • Answer questions about your medications, herbal supplements, and over-the-counter medication

Based on your doctors' orders, our team will deliver home medical equipment to your home.

Our home health aide will come to your home and help take care of you and your daily activity. Examples of what a home health aide will do are assisting your loved one with bathing, dressing, taking medications, or preparing meals. 

All members of our care team undergo extensive criminal background checks.

Our nurse-pharmacist care model allows us to prevent preventable harm by evaluating your medications and developing a customized medication program that may reduce your pill burden and reduce the side effects. This innovative program has found an average of 3.1 medication problems per patient (the patient was either had too low dosage, too high dosage or missing a dosage) and have saved 400 to 1,200 dollars in medication costs.
